A wire is poking your cheek. Here's how to fix it tonight

A poking archwire is the most common orthodontic complaint there is, and most of the time you can make it comfortable at home in a couple of minutes.

It is almost always the same story. Teeth have moved — which is the whole point — and as they shift, the archwire that was exactly the right length last month is now a millimetre too long at the back. That extra millimetre finds your cheek.

It is the most common thing patients call us about, and it is one you can usually settle yourself in a couple of minutes.

Try these in order

1. Cover it with wax. Dry the area with a tissue first — wax will not stick to a wet wire. Roll a pea-sized piece between your fingers until it softens, then press it firmly over the end. Reapply as often as you need to; you cannot overuse it. If you have run out, call and we will give you more.

2. Rinse with warm salt water. One teaspoon of salt in eight ounces of warm water. Swish for a minute or two and spit it out. This won’t move the wire, but if your cheek is already sore it takes the edge off and keeps the area clean while it heals.

3. Only as a last resort, clip it. If the wire is genuinely unbearable, you cannot get it to sit flat, and wax isn’t holding, you can use a small fingernail clipper to cut the wire behind the last tooth it is firmly attached to. Sterilise the clipper first, and hold gauze or a folded tissue around it so the piece you cut off doesn’t get swallowed. Then cover the new end with wax.

Do not reach for the clippers first. Try the wax — it solves most cases.

There is no need to feel like you are bothering us with this. Wires poking is a normal part of treatment working, not a sign anything has gone wrong.

About the sore spot

If the wire has already rubbed your cheek raw, that ulcer will heal on its own in a few days once the wire stops catching it. Salt-water rinses help. So does keeping wax on the wire until you are seen, even after it stops hurting — the spot needs a break to recover.

Cheeks and lips also toughen up over the first week or two of treatment. Early on, a wire that would not bother you at all six months from now can feel sharp. That settles.

When to call rather than wait

Get in touch promptly if:

  • The wire has come completely out of the tube on a back molar
  • You have swelling, not just soreness
  • Pain is severe and not responding to salt water or your usual pain reliever
  • You have swallowed a piece of your appliance and feel any discomfort breathing or swallowing
